Try this scenario. Same guy/club/group/syndicate owns two gliders. Now, it is usual to take the electrics out of a glider between flight as they are both valuable and desirable, lightweight and easily removable items. While in theory with two machines and two sets of transponders/radio/sat nav etc, they would be set up for the individual machines. However one set has been duff, so the other set changed to permit the glider to be flown. Then the repaired set is placed back in the other glider without checking that the transponder hex code is set-up correctly. So two aircraft with the same hex code flying at the same time.
As today seems to be a day to have a good snipe at my posts, who will be the first to come up with something better?
